The operational changes will leave millions of customers without key healthcare services.

Walgreens shoppers are likewise seeing service changes in the healthcare space[/caption]

The retail healthcare industry saw big changes in 2024 as massive corporations tweaked their healthcare strategies.

Increasing operational costs, low reimbursement rates for care, and shifting market conditions, such as an increased adoption of telemedicine and a greater consumer focus on convenience and affordability, have all played a role.

Balancing care services with traditional retail operations has proven challenging, requiring more than just financial investments to keep the retail healthcare model afloat.

Businesses are forced to align their approaches with healthcare regulations, deal with volatile reimbursement models, and confront staffing shortages, all while keeping profitability.

Such an uncertain financial landscape has several major retailers like Walmart, CVS, and Walgreens scaling back or tweaking their approach to retail clinics.

WALMART

Retail giant Walmart revealed last year its plans to shut down all 51 of its health clinics across five states and discontinue its virtual care services.

“Through our experience managing Walmart Health centers and Walmart Health Virtual Care, we determined there is not a sustainable business model for us to continue,” the company shared in a release last April.

Launched in 2019, the health centers were aimed at providing affordable healthcare to individuals in underserved communities.

They struggled to remain profitable due to reimbursement structures and rising operational costs.

Walmart, however, partnered with CenterWell last year, with the latter company plotting to open senior-focused primary care centers in 23 former Walmart Health locations by the first half of this year.


CVS

CVS has faced mounting financial difficulties in recent months, including rising costs within its Aetna health benefits business.

The company lowered its earnings outlook multiple times last year, pushed to lay off nearly 3,000 workers and outline plans to cut its expenses by a whopping $2 billion.

Part of its money-saving strategy included discontinuing some Coram infusion services and closing or selling hundreds of retail pharmacy locations.

CVS is also looking to no longer offer plans on the ACA exchanges under its Aetna brand beginning in 2026, a move that will affect the roughly one million people enrolled in Aetna ACA plans across 17 states. 

CVS closes 900 stores

CVS Health is finishing up its three-year plan to close 900 stores.

CEO Karen Lynch said it has shut down 851 so far, The Associated Press reported in August 2024.

The company announced the plan in November 2021, saying that the closures would lead to a retail presence that ensures it has the “right kinds of stores in the right locations for consumers and for the business,” per CNN.

The shutdowns amount to almost 10% of CVS’s footprint, in response to the shifting of “consumer buying patterns.”

The closures are part of a wider change in strategy of the company’s almost 10,000 locations.

Some stores have been removed to include more health services, such as primary care, and an “enhanced version” of CVS’s HealthHub layout.

“We remain focused on the competitive advantage provided by our presence in thousands of communities across the country, which complements our rapidly expanding digital presence,” Lynch said in 2021.

The shutdowns cost the company approximately $1 billion dollars in the fourth quarter of 2021, according to CNN.

Another big change seen over at CVS was the introduction of the new transparent pricing model called CVS CostVantage for its pharmacies.

The model was designed to bring greater transparency to pharmacy pricing by using a clear formula based on the drug’s acquisition cost, a set markup, and a dispensing fee.

WALGREENS

Walgreens is likewise reworking its healthcare strategy, moving away from solely relying on VillageMD, a national provider of primary care services that had many locations within or next to Walgreens pharmacies.

Walgreens previously invested heavily in VillageMD, spending over $6 billion to acquire a majority stake in 2021.

The company aimed to integrate primary care into its pharmacies, but financial pressures and rising operational costs pushed the retailer to shut down 160 VillageMD clinics in 2024.

Walgreens has also revealed its plans to close 1,200 retail locations over the next three years, making up roughly 14% of its total stores.

The drugstore chain is working to expand its virtual healthcare offerings to reach more patients across the country, providing care services online to 30 states as of October, Walgreens announced.

AMAZON

While these companies have taken strides to pull back from certain healthcare services, Amazon continues to invest heavily in the industry.

The e-commerce giant, for example, acquired One Medical in 2023, later announcing a new partnership with Cleveland Clinic.

The retailer also continues to offer Amazon One Medical for both in-person and virtual primary care as well as Amazon Pharmacy for prescription medications.

Amazon’s moves highlight the increasing role of both technology and partnerships in meeting consumer demand for accessible care.
